Form Instructions

There are two ways you can submit information for the Iberville Wall of Veterans:

1) You can download the form here and bring it, along with your photo to the library
2) You can complete the below online form 

The deadline to submit is Sunday, October 8, 2023. If you are using the online form, the photos should be submitted as a high-resolution jpg, not to exceed 10mb in size. Please DO NOT use commas within the photo file name.
