Public Records Request Submission Form
Fill out the following information to submit a Public Records Request.
- Don't put confidential information, like your social security number, in your public records request.
- If you don't know the name of the record that you are seeking, describe the information that you believe is contained in the record.
- Your public records request is a public document.
What happens after I submit this request?
Please include your contact information in case clarification is needed or to confirm payment for copies. Requests will be processed as quickly as possible; however, response times may vary based on the size of the request and how the records are stored. Some public records requests may also require legal review.

Fees
The Parish has established fees for certain filings, services, and publications.
The fee for public records is $0.25 per page. This fee applies regardless of whether records are provided in paper or electronic format. Each requester is also required to pay the cost of any storage medium used to provide the records.
